Talking about choices and care around the end-of-life can be difficult. What do we want? Who will decide for us if we cannot? If you are like most Americans, you have put off thinking or talking about these issues. Having these conversations before a crisis, when there is time for honest discussion and planning is more valuable for you and your family. In the morning session you will learn about your choices and how to share them with your loved ones from Michael Bordofsky, MD, Eileen Bunning, RN and Christopher C.
Jones, Attorney at Law. In the afternoon session you will receive a free copy of 5 Wishes, an easy-to-use advance healthcare directive and view the 5 Wishes DVD. A panel of experts including Reverend Teena Grant and attorneys Sharon Kennedy and Lori A. Lewis will answer questions and provide general legal and spiritual information. You may attend either morning, afternoon or both sessions. Parking is available in the structure on the corner of Pueblo and Castillo. Ticket validation will be provided. No host lunch available in Cottage Cafeteria. RN/LVN, CNA, MFT/LCSW 6 contact hours.
