Hospice Medical Director: Is board certified in Hospice and Palliative Medicine and can bring expertise in pain and symptom management when needed.
Registered Nurse: Generally acts as the Case Manager and ensures that your plan of care is well coordinated and meets you and your family’s unique needs.
Medical Social Worker: Assesses and develops a plan to assist with emotional, relational and financial needs. Provides supportive counseling.
Physical, Occupational and Speech Therapists: Provide expertise to ensure maximum physical functioning.
Certified Home Health Aide (HHA): Provides assistance with bathing and grooming to ensure comfort.
Dietitian: Is a resource to find foods that are appealing and nutritious, and that are easy to swallow and digest.
Spiritual Counselors & Chaplains: Serve in a nondenominational capacity to bring consolation and find meaning in life and death.
Bereavement Counselor: Provides support while exploring the meaning of death and experiencing grief.
